ADHD assessment is more than completing a questionnaire. Our process is designed to build a comprehensive picture of symptoms, history and day-to-day functioning before determining whether a diagnosis is appropriate and discussing treatment options.
Assessments are completed over a series of appointments, allowing adequate time for evaluation, discussion and individualized care
A detailed initial appointment reviewing current concerns, symptoms, medical and developmental history, and how these concerns affect daily life. Standardized ADHD assessment tools are incorporated as appropriate.
Adult (Ages 18+) - $430.00
Pediatric (Ages 13-18) - $445.00
Your Nurse Practitioner reviews the assessment findings with you, discusses whether diagnostic criteria are met, and develops an individualized treatment plan when appropriate.

A follow-up appointment to assess progress, response to treatment and any concerns or adjustments that may be needed.

Continued follow-up to monitor symptoms, treatment effectiveness and overall progress. The frequency of future appointments is individualized based on your needs.

No referral is required to book an ADHD assessment.
Yes. If you have an established ADHD diagnosis, our Nurse Practitioners can provide ongoing treatment and medication management. Please provide medical documentation supporting your diagnosis so we can review your previous assessment and treatment history.
You may not need to repeat the comprehensive assessment. With appropriate documentation confirming your diagnosis, we can often begin at Appointment #2: Diagnosis & Treatment Planning to review your history and discuss treatment options.
To skip the initial comprehensive assessment, we require medical documentation supporting your ADHD diagnosis. This allows our Nurse Practitioners to confirm the previous assessment and safely continue or initiate treatment. If adequate documentation is not available, a new comprehensive assessment may be required.
No. The purpose of the assessment is to determine whether your symptoms and history are consistent with ADHD and whether another explanation should be considered.
Treatment recommendations are individualized. When medication is clinically appropriate, options can be discussed with your Nurse Practitioner as part of your treatment plan.
Documentation may be available when clinically appropriate. Documentations fees may apply.
